Diabetes Care
Kenmore Clinics provides support for Diabetes Care & Management.
Kenmore Clinics is committed to providing high quality and comprehensive care for people with all types of diabetes.
After completing the rigorous application and assessment process of the Australian Diabetes Educators Association (ADEA) credentialing program, one of Kenmore Clinic’s Registered Nurses, Nicole McClure, was awarded the status of ‘Credentialed Diabetes Educator’ (CDE) in June 2019.
CDE’s are specialists in diabetes and can be the first point of call for people wanting more information, motivation or support to manage their diabetes. They have an in depth knowledge of all aspects of diabetes and can recognise when you may need to see other members of your health care team and can link in with other health professionals.
Nicole can work with you to establish a management plan for your diabetes. Diabetes is a condition that can be comfortably lived with. It just requires some additional care and management.
What's included as part of your Diabetes Management Plan?
- Regular monitoring of your condition
- Diabetes education
- Allied health referrals (psychology, physiology, physiotherapy)
- Medicaiton prescription, assessments & reviews
- A helping and supportive hand as you navigate this new condition and its impact on your life.
Why choose our General Practice?
Understand your condition
Our GPs believe in the power of knowledge. Particularly when you're navigating a condition that's completely new to you, it can be so beneficial to learn all about what it means for you and how to care for it.
A proactive approach to your care
We ask you to come in every six months for a check-up so we can monitor your condition. This allows us to identify any issues or concerns early and treat them before they become more serious concerns.
Open 7 Days
We're committed to providing you with the very best health care services. As part of this, we open seven days a week and offer extended opening hours to ensure you can always make it to your appointment.
We work with your wider team
Depending on the extent of your diabetes, you'll have an allied health team made up of several medical professionals. Each discipline brings its own perspective and priorities to your care. We'll work within this team.
